Registration via Intake Form
In order to use this website to book an appointment, a user must first complete an "intake" form. During registration a user is required to give certain information (such as name and email address, age, health status, and any screening information needed such as personally identifiable information and employment/self-employment information). This information is used to contact you about the services on our site in which you have expressed interest, and to screen and verify you to make sure you are safe for Nurse Gypsy & Associates to book an appointment with.
We request information from you when you are booking an appointment and issuing your required non-refundable deposit. To book with Nurse Gypsy & Associates, you must provide contact information (like name and email address) and financial information (like credit card number, expiration date, CVV code and zip code). This information is used for billing purposes and to hold your appointment. If we have trouble processing a deposit, we'll use this information to contact you. For your privacy and security, your financial information is destroyed after each transaction.
We share aggregated demographic information with our partners and advertisers. This is not linked to any personal information that can identify any individual person.
We use a outside credit card processing companies to bill users for deposits. These companies do not retain, share, store or use personally identifiable information for any secondary purposes beyond filling your order.
